PARTNERING FOR YOUR SUCCESS

Understanding, navigating, and activating an NDIS Plan can be a challenging endeavour. This is further complicated when identifying informal supports and strengthening formal relationships, as well as investigating mainstream options not funded by NDIS.

At FCS we want to partner with you in the following ways:

  • Identify options for you (informal, mainstream, and funded).
  • Research and connect you with services that are innovative that do meet your expectations and needs.
  • Understand your budget’s flexibility and support you if a review is needed with NDIS.
  • Make decisions concerning providers by establishing service agreements and terms/conditions of supports.
  • Monitor your spending and the quality of your service providers.
  • Address personal barriers that are preventing your participation with community.
  • Resolve and advocate for you in relation to service delivery concerns.
  • Participate and activate NDIS reviews and support you through this process.
  • Technical advice in relation to navigating MyPlace portal.
  • Reflect on what is working and what needs to change, and prepare for NDIS reviews.
  • Build your capacity in understanding NDIS so you can independently manage your plan in the future.

GET THE RIGHT LEVEL OF SUPPORT

We offer three levels of support connections and coordination.

Level 1 Support Connections

Time limited assistance to strengthen your ability to connect with informal, mainstream, and funded support, and to increase your capacity to maintain relationships, resolve service delivery issues and participate independently in NDIS process.

Level 2 Coordination of Supports

Assist to strengthen your ability to connect to and coordinate informal, mainstream and funded supports in complex service delivery environments. This includes resolving points of crisis, developing capacity and resilience within your network and coordinating supports from a range of sources.

Level 3 Specialist Support Coordination

Our Specialist Support Coordinators work within an NDIS specialist framework in accordance with high level risk presented by a participant’s situation. These can involve matters concerning the justice system and complex psychiatric conditions that are likely to place a person at risk of homelessness. The support provided is timely and focus on addressing barriers to reduce complexity and support. Participants connect with services to build their capacity and enhance their lives. Our role is also to coordinate specialist disability support workers to support participants with mental health.
